Step 1 : Passenger Information
1.Total Number of Passengers... (1) if you are a Single or if you are rooming with someone but only completing the form for yourself at this time (2) if there are 2 of you in the cabin and you're completing the information for both of you at the same time (3) if there are 3 of you in the cabin and you're completing the information for everyone at the same time 2.Passenger Name... If you have a Middle name you must put that... no Initials. Leave blank if no Middle name. See "Required Travel Identification" if you have any questions 3.Gender... must be filled in
Step 2 : Contact information
1. Whose Address is this?... If you are completing the form for multiple people and you have different addresses... indicate who's address it is by changing from "Passenger 1" to "Passenger 2", etc. 2. Email & Phone... complete the same as the Address field
Step 3 : Trip Information
1. Resort/Cruise box... don't need to put anything 2. Travel Dates... Jan. 18-21, 2019 3. Cabin Type... are you choosing an Interior, Oceanview or Balcony room 4. Bedding... choose "King" for 1 bed or choose "Double" if you need 2 beds 5. Insurance... Travel Protection Insurance, if desired, must be purchased NO LATER than the day you make your final payment. Travel Protection Insurance is always HIGHLY recommended, however optional. If you choose NO... it is implied you have read the Cancellation Penalty section and are aware of the penalties... and that you are willing to take the chance without Travel Protection Insurance 6.Group Name... put No Sleep Nassau 7.Booking Number... don't need to put anything
STEP 4 : Payment Information
1.Payment Type... indicate which Credit Card type you will be using first 2. Complete the Card Number, Expiration Date, Name on the Card, Billing Address (with City, State & Zip) 3.Payment Description... I need you to put the CVV Number (3-digits for Visa & MasterCard on the back of your card or the 4-digits for AMEX on the front of your card). Per Carnival's regulations, payments cannot be processed without the CVV number 4.Pay in Full or or Deposit Only... If you are choosing Payment Option #1, check Deposit Only. If you are choosing Payment Option #2, check Pay in Full 5.Submit an Additional Payment Type... If you are entering credit card information for multiple people... check Yes... then start the whole process over again for the next person, starting with Payment Type.
If using a Debit Card... in order to not receive a "Denied Charge", the traveler must contact their Bank to verify what their Charge Limit is for 1 day. This is the maximum amount allowed to be charged for all charges in that day.
As indicated in the "Payment Option" boxes... the 2nd and Final Payments will be processed on those specific dates. It is the traveler's responsibility to inform the Travel Agent (Chrystal Kowack) of any changes to the card number, expiration date or CCV number prior to the scheduled payment dates.
STEP 5 : Additional information
1. Special Requests... If you are rooming with someone, and you indicated "1 Passenger" in step 1... this is where you'll need to put your roommate's name. Or I will have to assume you are paying for a Single room. 2.Questions/Comments... freeform, whatever you have questions about

As mentioned above, the Travel Protection Insurance is not included in your cruise total
The Protection Insurance is available for purchase NO later than the final payment date... and is completely Non-Refundable once purchased
For Cruises there are multiple Identification options available
1. Non-expired Government issued Photo ID + a Certified Copy of your State issued Birth Certificate 2. A Passport Card 3. A Passport Book (compare a Passport Card to a Passport Book)
Both a Passport Book and a Passport Card... Must be valid for at least 6 months past the return date.
If a Passenger's Full Legal Name is not provided, exactly as it appears on their Passport or Driver's License, no later than prior to theFinal Payment... there will be an additional Travel Supplier penalty fee of $25 per name revision plus any possible penalties
Transfers... if you are flying into Miami the first day of the cruise and departing the last day of the cruise (ONLY)... here is information about Carnival's Airport Shuttle Transportation
